RCA is an important tool for companies that are seriously focused on creating a learning environment and reducing operational risk. RCA is a methodology which can be used to systematically uncover the reasons behind a failure event. It investigates patterns of negative effects to find hidden flaws, and the specific contributing actions. Whether these actions are physically-related or due to human or organisation behaviour, there is often more than one root cause (a complex combination of several factors). Our interactive workshops brief participants on how RCA works, who apply this theory to case studies and real-life examples. Participants also review and give feedback on existing organisational processes on incident management, design enhancements and training.
